Intervention Questionnaire
If you think your loved one has a problem with alcohol or drugs and Intervention is necessary, here are 22 questions to help you decide:

Have you or your loved one lost friendships due to their use of alcohol/drugs andrelated dysfunctional behavior/codependency?

Does your loved one seek to escape their problems through drugs/alcohol and/orrelated dysfunctional behavior?

Does your loved one ever use drugs/alcohol to feel socially accepted?

Does your loved one use drugs/alcohol to enjoy themselves?

Has your loved one’s health ever been affected through the use of drugs/alcohol?

Has your loved one ever been in a Hospital or Institution because of drugs/alcoholand/or related dysfunctional behavior?

Has your loved one ever been arrested while Under the Influence of drugs/alcohol?

Has your loved one ever taken an overdose, or, had a blackout due to drugs/alcohol?

Do they ever use drugs/alcohol to build selfÂconfidence?

Do they ever deny or hide their use of drugs/alcohol?

Have their normal functions ever been affected by the use of drugs/alcohol?

Did people lose respect for your loved one due to their drug/alcohol use?

Have they ever engaged in Illegal Activities in order to get drugs/alcohol?

Have they ever expressed feelings of guilt or remorse because of their use ofdrugs/alcohol?

Have they ever tried to stop using drugs/alcohol but could not?

Has your loved one ever tried to control their use of drugs/alcohol?

Is the use of drugs/alcohol making your home life unhappy?

Has/does your loved one’s personality change as a result of drugs/alcohol?

Have you or your loved one ever lost time from school or work due to the use ofdrugs/alcohol and/or other related activities?

Have you ever moved from place to place in an attempt to stop their usingdrugs/alcohol?

Did you ever attempt to keep your loved one away from bad influences, i.e. drug andalcohol environments?

Have you ever been hospitalized as a result of your lovedÂones drug/alcohol useand/or related dysfunctional behavior?
Through our personal experience with drugs/alcohol, we have found that if you answered yes to 1 or 2 of these questions, your loved one may have a drug/alcohol problem.
If you answered yes to 3 or more of these questions your loved one definitely has a problem with drugs/alcohol.
